Advance Care Planning is a process of reflection and communication. It is a time for you to reflect on your values and wishes and to let people know what kind of health and personal care you would want in the future if you were unable to speak for yourself.
This workshop will help you to identify who your Substitute Decision Maker is and know what your rights are in terms of Ontario's Health Care Consent Act. There will also be tips and tools shared about how to start ACP conversations with your people. Now is the time to prepare for the future. It is always too early until it is too late.
Amy Pritzker has worked in Hospice Palliative Care for the past 13 years. She has been part of the Community Support Team at Hospice Simcoe for the past four years, serving as a Bereavement Coordinator at the Bell Farm Location. She is also part of the Advance Care Planning Champions Community of Practice through Hospice Palliative Care Ontario.
Amy received a Bachelor of Arts in Psychology and a Bachelor of Social Work from McMaster University and completed her Master’s in social work through the University of Victoria. In 2005 she completed the Spiritual Formation program through Carey Theological College in Vancouver, BC.
Before working in Hospice Palliative Care, Amy worked in Housing, Child Welfare and Domestic Violence. In her current role at Hospice, she provides information to individuals, groups and organizations to help them navigate the messiness of illness, dying and grief. She also facilitates groups for grieving individuals, supporting them as they move through loss and trauma into post-traumatic growth.
